Job Description

The Panel Manager’s role is to assure quality care and access to our chronic disease patients. The primary role of the Panel Manager will be to coordinate patients’ access to services, facilitating follow up and acting as a liaison between healthcare clinicians, patients, and families. Secondary responsibilities include administrative management of data collection; documentation and reporting once primary responsibilities are maintained or accomplished.

Qualifications

Qualifications:  Minimum of High School Diploma or GED Equivalent plus relevant experience. Bachelor’s degree preferred. Prefer, but not required one (1) year of experience in direct patient care and/or medical assistant certificate.  Experience working in community health care setting preferable.  Bilingual in English and Spanish and/or other languages preferred.  A thorough understanding and working knowledge of patient confidentiality issues and HIPAA compliance/regulations is essential
