Augustana Regent at Burnsville is seeking a full time Director of Social Services and Resident Services for our extraordinary senior living community that caters to the lifestyle needs and wants of people 55 and over. We are seeking someone who has experience in meeting high standards in the areas of interpersonal communications and ensuring that resident’s daily living needs are met.
Job Description
Job Duties:
- Coordinate social services activities with all other facility departments.
- Maintain necessary documentation in resident's progress notes.
- Schedule and participate in care conferences for residents as needed to provide residents, families, and staff with continuity in communication and effective problem solving.
- Serve as liaison between the resident and the facility regarding the resident's needs including assisting residents and families with personal clothing and care needs, roommate tensions, and financial counseling.
- Oversee and complete referrals for resident mental health services and coordinates consultative services of mental health provider.
- Meets state and federal regulations for social services tasks regarding resident care.
- Assist residents annually in their Medicare Part D selection.
- Assist in planning and monitoring the department budget.
- Assist with special events during the year.
- Participate in Manager's meeting, Interdisciplinary Meeting and other internal meetings as directed.
- Perform other tasks as assigned.
Qualifications
The successful candidate must have:
- Degree in Social Work
- Two to five years experience in senior housing is preferred
- Good organizational, leadership and interpersonal skills
- Be self-motivated along with the ability to multi-task
- Knowledge of Eldermark software is preferred
- The ability to coordinate services internally and externally
- Knowledge of waiver programs and veteran’s benefits would be preferred
- Marketing experience in senior housing would be preferred
