Job Description

In collaboration with the clinical manager, act as internal case manager and advocate for clients.


(a) Participate in Person Centered Planning processes. 


(b) Collaborate with the client, family, case managers, Habilitation Techs and other members of the multi-disciplinary teams to implement and evaluate client care needs and individual goals identified in the Person Centered/Individual Support Plan. 


(c) Make regular client visits, as per policy, to evaluate client progress and to monitor that client services are implemented according to the Person Centered/Individual Support Plan and the service definitions. 


(d) Evaluate the progress of goals identified in the Person Centered/Individual support plan. 


(e) Maintain ongoing communication with case managers/care coordinators. 


(f) Provide complete, accurate, and timely documentation in the client chart, home chart and computer system.


Provide oversight to field staff: 

(b) Participate in the hiring of field staff and interview process, as applicable. 


(c) Participate in new hire orientations. 


(e) Participate in client specific orientations as directed by the Clinical Manager. 


(f) Provide feedback to employees including recognition, counseling and termination of employment. (g) Provide input to the client services manager and clinical manager for completing field staff performance evaluations. 


(h)Function as a liaison between field and office staff.



Support the team (office and company): 


(a) Provide guidance to office colleagues to promote understanding of and response to the needs of clients with intellectual/developmental disabilities and related conditions. 


(b) Represent BAYADA in community and marketing activities. 


(c) Participate in Performance Improvement activities. 


(d) Participate in office and company meetings, events and functions.




Assist in the development, management and implementation of Habilitation services and contracts by maintaining positive relationships with area referral or funding sources.


Perform related duties, or as required or requested by supervisor

Qualifications

Bachelors Degree Required

Demonstrates record of strong interpersonal skills.

Prior supervisory experience.

Demonstrated record of goal achievement.

Demonstrated record of successfully taking on more responsibility with positive results and ambition to grow past current position.

Demonstrated record of strong interpersonal skills

Competence in basic PC skills required to perform job functions.

Ability to read, write and effectively communicate in English.
