CentraCare is part of the National PACE Association (Program of All-Inclusive Care for the Elderly). We are a medical/social-based model of care that provides comprehensive, integrated managed care to frail older adults who are at risk for nursing home placement. Through interdisciplinary team planning and coordination, older adult participants receive coordinated care as they age in place at home. All services and products covered through Medicare and Medicaid are provided by CentraCare PACE. 

Job Description

To oversee the daily operations for Social Work and Clinical Care Coordination in the home or alternative care setting. Duties include ensuring compliance with all Centers for Medicare and Medicaid Services (CMS) and Michigan Department of Community Health (MDCH) guidelines and regulations. The Center Director may direct your duties to ensure that the highest degree of quality care is provided and maintained to all participants.


• Facilitate morning team meetings and alerts the Quality Coordinator of all possible level II instances, potential participant disenrollments and grievances. 
• Collaborates with other managers to establish and maintain consistent clinical care delivery for participants
• Leads and coordinates care plan meetings, semi-annual, annual reassessments and significant change assessments with IDT.
• Meets at least monthly with the Clinical Director to discuss employee relations, organizational culture, and the strategic plan including goal identification and progression, and review department budgets.  May be asked to develop and maintain reports or spreadsheets.
• Prepares periodic reports to review staff performance as related to planning, caseload allocation, and quality of work, procedures, and participant care.
• Functions as an active member of the IDT and Management Team. Serves as a key communicator between the Operations and Clinical Director and reporting staff.
• Serves on and regularly attends assigned committees, including but not limited to: Utilization Review and Falls Committee/team. Participates in organizational QI activities. May be asked to represent the Clinical Director at other committees or community outings as necessary.
• Assists with the grievance and appeals process as requested by the Clinical Director. May be asked to serve as the communication liaison with participants, family, and vendors as needed.
• Assures the mission and vision for CentraCare are adhered to, in accordance with organizational strategic plan and assists guiding long-range objectives.
• May be requested to assume a case load up to 30 participants as requested by the Clinical Director and provide coverage for SW time off.
• Continuously works for improvement of CentraCare, recommending and supporting new ideas/suggestions to the IDT and staff. 
• Uses own knowledge and experience base along with other resources as necessary to make logical decisions and solve problems.
• Maintains stable performance and emotions when faced with opposition, pressure, and /or stressful conditions.
• Understands and appropriately applies the chain of command in relation to job position and supervision.
• Assists the Clinical Director with development of an overall business plan and budget for reporting departments.
• Conducts performance evaluations and disciplinary action for direct reporting staff.
• Coaches employees as necessary to ensure the team is functioning at a high level of performance.
• Collaborates with the Clinical Director to adapt and review all policy/procedure/protocols related to assigned departments and ensure ongoing staff compliance.
• Assists and supports the Clinical Director in training and coaching operational supervisors and team leaders. This may include facilitating/ teaching classes or course work.  New programing must be approved by the Clinical Director prior to initiation and must correlate with the organization’s strategic plan, vision and values.
• Responsible for assisting the Clinical Director and HR with new hires to include employee selection, pay determination, roles and responsibilities, and FTE management. 
• Responsible for coordinating necessary inservices and educational opportunities for direct reporting staff/departments.
• The Community Support Services Manager is expected to respond to and participant in after-hour emergent conditions that are deemed such by the CEO or Operations Director.

Qualifications

 

• Bachelor's Degree in a health field with a major in management or administration, or any combination of experience, education, or training which would provide the knowledge required by this position.
• Registered Nurse or MSW preferred.
• Minimum of one year of experience working with the geriatric population.
• Must have proven leadership experience and skills with the ability to get along harmoniously with others in a team environment.
• Must possess the ability to plan, organize, develop, implement, and interpret goals, objectives, policies and procedures necessary for providing quality care.
• Must have patience, a cheerful disposition and enthusiasm.
• Must be knowledgeable of computers, data entry, output, system applications, etc.
• Must have reliable transportation and current valid Michigan driver’s license.
• Immunizations and vaccines must be current or medical documentation must be provided from a medical provider with reason (s) to request waiver.  (Hepatitis B, Td, PPD specifically)
