CentraCare is part of the National PACE Association (Program of All-Inclusive Care for the Elderly). We are a medical/social-based model of care that provides comprehensive, integrated managed care to frail older adults who are at risk for nursing home placement. Through interdisciplinary team planning and coordination, older adult participants receive coordinated care as they age in place at home. All services and products covered through Medicare and Medicaid are provided by CentraCare PACE. 

Job Description

Key Responsibilities and Accountabilities:  


Develop and implement the home care services for CentraCare participants, including identification of needed durable medical equipment and nursing, homemaking and personal home care services. 

• Conducts initial and periodic assessments, identifies, and develops a specific plan of care for the home care needs of CentraCare participants and assures all are completed as described in CentraCare policies.
• Conducts home assessments with focus on safety issues related to potential falls, fire safety, cooking safety, oxygen therapy safety, etc.  Educate participants on safe practice and environment.
• Completes home MDS (Minimum Data Set) information in Truchart for state reporting.
• Covers duties of the Care Coordination Supervisor in his/her absence. Such as Developing, coordinating, schedulings, and supervising the implementation of all home care services, which includes nursing, homemaking and personal care services to ensure that quality care is provided to meet participant needs, including in a Skilled Nursing Facility, according to CentraCare standards. 
• Creates and distributes home care PCA assignments in the absence of the Care Coordination Supervisor.
• Coordinates and identifies needed durable medical equipment for participants.  Notifies staff which orders DME of participant DME needs.
• Follows participants requiring complex wound care in the clinic and documents weekly progress and treatment changes.
• Functions as a member of the IDT, communicates participant changes to team members, participates in IDT meetings, and the development of the participant plan of care.
• Participates in the coordination of the participant’s 24-hour delivery of care services.
• Completes and processes grievance (complaint) forms as appropriate.
• Maintains a computer database of the Home Care Department data and statistics.  Provides summary reports, both as requested and routinely required reports.
• Participates in Quality Improvement activities.
• Documents in the participant’s medical record as required by policy.

• RN–Nursing Diploma or Degree from accredited School of Nursing or University. 
• Minimum of one year of experience working with the frail, elderly, or long-term care population.
• Must possess a current, unencumbered, active license to practice as a Registered Nurse in the State of Michigan.
• Must have reliable transportation and current valid Michigan driver’s license.
• Possess current CPR certification by AHA or willing and able to obtain as offered by CentraCare.
• Immunizations and vaccines must be current or medical documentation must be provided from a medical provider with reason (s) to request waiver.  (Hepatitis B, Td, PPD specifically)
