Job Description

Our Care Coordinator for Health Homes is responsible for the overall administration of the HH teams at the assigned site.  This includes clinical supervision of HH staff, coordination of client services, staff recruitment and hiring, staff training, community and case finding, weekly, monthly and quarterly reporting of site information to the Deputy and Program Director, and implementation of the program policies and procedures. Responsible for site delegation of rosters, monitoring and reporting of the tracking sheet and DOH matrix.

Qualifications

  • Masters degree in Social Work preferred or Masters level with commensurate clinical training and experience. 
  • Three (3) or more years postgraduate experience
  • Two (2) years of supervisory experience
