Contra Costa Interfaith Housing (CCIH) is a vibrant and socially responsive non-profit agency which provides permanent housing and vital support services to approximately 800 people in Contra Costa County who are homeless or are at-risk for homelessness. Over 35 faith-based communities support our work, although there are no religious requirements or affiliations required to receive services.

Job Description

Contra Costa Interfaith Housing (CCIH) is seeking a Case Manager to join our East Contra Costa County team. This position is part-time, non-exempt and will start at 32 hours per week, but may become full-time over the next year. Some weekend and evening hours may be required. This position is entitled to benefits including a generous paid time off package, health insurance and a 403B plan.

The Case Manager will be responsible for delivering support services to families living in two affordable housing programs in East Contra Costa County (Bay Point and Pittsburg). Services will include assessment, information and referral, coordination, and development and implementation of resident plans in order to support their highest level of self-sufficiency, parenting, and health. The Case Manager will work as part of a multi-disciplinary on-site team with the Youth Enrichment Coordinator, the Director of Support Services, and the Academic Supervisor. The Case Manager will also help plan and oversee social/educational activities at these sites, attend community meetings, and form collaborative relationships with service providers in Contra Costa County. Familiarity with the resources in East Contra Costa County and bi-lingual/bi-cultural skills preferred.   

Specific responsibilities include:

• Maintain well-defined linkages and working relationships with service providers that meet specific needs of the residents. Essential services include but are not limited to:

o Health care (including HIV/AIDS services)

o Education and employment development

o Children’s services such as after school programs, tutoring, child care, medical and mental health resources

o Housing retention services

o Child and family support services including educational, recreational, financial, employment, medical and mental health resources  

o Alcohol and other drug addiction prevention services 

• Work closely with the service team and with property management to support residents to overcome issues that may place them at risk for eviction. These issues may be related to: 

o behavioral issues, 

o financial problems, or 

o various challenges that affect the resident’s ability to abide by their lease agreement/rules of tenancy.

• Develop resident engagement and community development strategies, in conjunction with the service team, which encourage them to take advantage of available services and programs.

• Provide case management services to residents in need. Case management services include: 

o intake assessment including development of need specific goals

o service coordination

o follow-up

o support, and

o advocacy as needed

• Transport residents as needed to vital appointments and services utilizing Case Manager’s automobile.

• Provide an information and referral service for residents in need of special services with follow-up to ensure that access to those services was achieved.

• Maintain tenant files and documentation in accordance to CCIH policies and procedures and HIPAA regulations.

• In collaboration with the Volunteer/Outreach Manager and the Director of Support Services, coordinate on-site educational forums and activities for the residents, as agreed upon by the service team.

• In collaboration with the Youth Enrichment Coordinator, develop a coordinated approach to addressing the needs of families and youth that are on your case load.

• Represent CCIH at service related meetings as directed by the CCIH Executive Director and the Director of Support Services.

• Ensure the highest degree of professionalism and quality practice in rendering services to clients served by CCIH. This includes quality assurance evaluation, action plans, and coordination of services as deemed appropriate to meet resident needs.

• Assist the outreach and development team with community outreach and fundraising for support services.

• All other appropriate duties as needed and determined by the service teams and Director of Support Services.


Qualifications

• Bachelor’s Degree in Social Service field or comparable experience required.

• Experience working with low-income families who have multiple challenges and competency working with culturally diverse populations required.

• Familiarity with Contra Costa County resources and communities a plus. 

• Must successfully pass a LiveScan screening and TB test.

• Ability to sit for up to 2 hours without a break.

• Ability to perform repetitive movements, such as typing and filing, and use of commonly used office machines and supplies.

• Ability to lift and move a minimum of 25 pounds including carrying a laptop computer.

• Ability to drive a car and transport oneself between meetings, including driving at night.


Additional Information

• This is a 32 hour per week exempt position, but may increase to full-time in 2016. Benefits are included.

• Compensation is contingent upon skill and experience.
