Job Description

Provides and coordinates excellent quality care and healing to Graham Windham’s children and ensures that every one of the children on the caseload is living in a nurturing, safe, and permanent family as quickly as possible

 

Duties & Responsibilities (but not limited to):

  • Recognizes that each child, parent and foster parent has strengths and is able to change.
  • B2H Behavioral Specialist provides Waiver Services designed to support clients and caregivers through building of skills to manage the client’s SED/DD condition.  The Specialist will create and implement a comprehensive behavior management plan with the youth/family and stakeholders to support adaptive functioning in a community setting.
  • Ensures that each child is meaningfully engaged in their education
  • The Behavioral Specialist works closely with Crisis Management staff to develop and implement a Crisis Management Plan for the youth and families served in Waiver Services Program.
  • Participates in weekly supervision with B2H Supervisor to ensure that services rendered are appropriate and in keeping with the client’s Individualized Health Plan (IHP).
  • Collaborates with B2H Supervisor, Health Care Integration Staff, Case Planning Staff and stakeholders (including resource parents, birth parents and others) as needed to provide feedback for Revised IHPs.  The specialist assists in tracking whether clients have met goals established in prior IHPs (efficacy), and assesses whether the goals have been met within established time frames.  The specialist   also participates in adapting goals/objectives in the Individualized Health Plan when needed, or in response to changes in client’s functioning.
  • Completes documentation of Detailed Service Plan/Service Summary Forms and Progress Notes in a timely fashion which meets regulatory guidelines for B2H Program. Specialist ensures that documentation accurately reflects the client’s progress related to his/her Individualized Health Plan.
  • Participates in Team Meetings as needed for each child/family serviced.
  • Participates in Training for B2H services as prescribed by OCFS guidelines.
  • Follows guidelines for Mandated Reporters inNew YorkState.
  • Performs other duties as assigned.

 

 

 

 

Qualifications

B.A. and B.S. in Human Services, Social work or related field. At least 2 years of applicable experience in a human services field providing client-centered services. Ability to manage multiple deadlines, remain calm and composed under stress, manage crisis situations. Familiarity with ACS, OCFS, OMRDD, NYS Medicaid, DYCD and other relevant municipal and state agencies.  Ability to complete the necessary documentation for billing and service planning and to meet the regulatory guidelines set by ACS/OCFS/NYS Medicaid. Ability to work with diverse populations (including LGBTQ) and cultures, process information and respond appropriately. Willingness to travel in within the 5 boroughs ofNew York Cityand Genuine Love for working with children, teens, families and foster families. Bilingual Preferred
