Job Description

Responsibilities include:

  • Provides pharmaceutical care to all ages of patients from newborns to the elderly. 
  • Reviews prescriptions issued by physicians or other authorized prescribers
  • Compounds medications using standard formulas and processes, such as weighing, measuring and mixing ingredients
  • Monitors all posting onto the patients profiles
  • Checks patient medication cassettes prior to delivery to patient care areas to ascertain that the correct drug, dosage form, schedule and strength is being dispensed
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors
  • Supports and maintains a culture of safety and quality
  • Prepares IV admixtures and extemporaneous parenteral products under aseptic conditions.
  • Answers questions and provides information to physicians, nurses, patients and other healthcare professionals on the appropriate use of medications, drug interactions, side effects, dosage, storage, etc
  • Ensures the maintenance and proper operation of equipment and supplies in the work areas
  • Maintains established procedures concerning quality control, security of controlled substances and disposal of hazardous waste
  • Treats patients and their families with respect and dignity.  Identifies and addresses pyschosocial, cultural, ethnic and religious/spiritual needs of patients and their families
  • Insures compliance with all governmental, professional and hospital rules and regulations regarding the practice of pharmacy
  • Maintains confidentiality and security when dealing with health information
  • Complies with organizational policies regarding ethical business practices
  • Performs other duties as assigned or requested

Qualifications

Qualified candidates will possess the following:

  • Current New York State Pharmacy license
  • 3 or more years related experience, hospital setting preferred
  • Available for rotating shifts, weekends and holidays
  • Ability to flexibly respond to changing demands
  • Demonstrate clear written and oral communication skills and interpersonal skills to deal effectively with other healthcare professionals, staff and patients
  • Ability to demonstrate good assessment, care planning & critical thinking skills
  • Skill in appropriate use of universal precautions, safe workplace and confidentiality methods
