Job Description

The Service Coordinator/Home Manager will coordinate and implement individual plans for 8-10 individuals.  Communicate on a daily basis to Interdisciplinary Teams.  Provides total service coordination to meet the needs of individuals.  Works closely with nurses and the Assistant Home Manager.  Supervises the operations of the home.  

This is a full-time position.  Days scheduled to work vary based on the home needs, may work evenings, weekends and holidays as needed to meet the needs of individuals and families. 

Qualifications

Bachelor's degree in human service field, required.  Must have one year experience in developmental disabilities field. 


Must possess excellent communication and organizational skills.  Must be able to problem-solve and have the ability to work independently.  Must be able to use Microsoft Office programs effectively. Must be able to prioritize successfully and meet deadlines.
