Highland Chateau Health Care Center is St. Paul’s landmark for rehabilitative and skilled nursing care. Every facet of Highland Chateau is designed to support a personalized, comfortable experience; one that helps residents recover, and return home, as quickly as possible. We have one of the region’s most comprehensive rehabilitation programs, which includes physical therapy, occupational therapy and speech therapy. 

Our newly renovated, 64-bed facility features amenities such as a beauty/barber shop and dining rooms on each level. We are conveniently located near shopping destinations, restaurants, public transportation, Minneapolis-St. Paul International Airport and the Mall of America. 

Job Description

Fulfill the oversight of  all social service programs for our skilled-nursing facility clients, including caregiver support, maintenance of ties with residents, families, and other groups.

 Interfaces with the admissions coordinator so as to understand the needs of new residents and their families.

 Greets newly admitted residents and their families to answer questions they may have and to facilitate adjustment to placement at Highland Chateau.

 Prepares a social history of the resident shortly after admission and helps establish social service goals for the resident.

 Makes appropriate entries in the resident’s medical record relating to social services.  Communicates resident needs to appropriate members of the care-giving team.

 Completes the appropriate sections of the MDS and the care plan relating to social services.

 Maintain a social services file on each resident.                                                                   

Receives resident complaints and facilitates the complaint process.  Interviews residents and/or family members regarding complaints and communicates all necessary information to other staff members. Confer with Director of Nursing or other Dept. heads regarding  residents' SATISFACTION or DISSATISFACTION with nursing cares or other services

 Coordinates the resident care conference schedule, creates a calendar with the help of other appropriate staff, and communicates the care conference schedule to other staff, the resident, and appropriate members of the resident’s support team.  Invites residents and appropriate family members to the care conference.

Maintain a liaison with hospital discharge planners and other social workers in the community.

 Attends facility department head and other meetings as required.

 Presents inservices to staff on occasion.

 Keeps track of hospitalized residents while in the hospital and communicates regularly with the hospital staff.

 Coordinates the resident discharge process, including the arranging of any necessary supportive in-home services.  Communicates all aspects of the discharge process to other staff.

 Represents the facility to the public through various functions.

 Other duties as required.

 

 

 

Qualifications

  1. Must have good physical and mental judgment;   possess mature judgment; have an out-going personality and be able to relate effectively with the residents, their families, the staff, and outside agencies.  Must have a genuine interest in working with and concern for senior citizens and persons who are disabled.

     

    Must have a Bachelor’s degree in Social Services (or a related field) and must be a Licensed Social Worker in the State of Minnesota, with experience in Social Work in a long-term care facility.
