The nurse care coordinator is a core member of the care team, including the patient’s medical provider, psychiatric consultant, as well as the larger primary care team or medical team. The nurse care manager is responsible for supporting and coordinating the addiction health care of patients on an assigned patient caseload with the patient’s medical provider and, when appropriate, other mental health providers.

What is required...

• Graduate of an accredited nursing program & currently licensed in the State of Washington
• Current CPR card (maintenance required)

What is preferred...

• Previous experience working in a clinic setting
• Working knowledge of addition of evidence based addition treatments and brief behavioral intervention

Wage:  Wage range starts at $27.21/hour, actual starting wage will depend on years' of applicable experience. 

FTE:  1.0 = 40 hours/week

Schedule:  Primarily Monday-Friday; 8:00a - 5:00p
