KVH Family Medicine – Cle Elum is a well-established practice housed in a beautiful, recently-expanded facility.  The practice is supported by on-site lab and imaging services, an electronic health record, orthopedic and physical therapy outreach visits from the main campus in Ellensburg, integrated behavioral health, and a seven-day-per-week Urgent Care Clinic.

Job Description

The behavioral health care manager is a core member of the collaborative care team, including the patient’s medical provider and psychiatric consultant, as well as the larger primary care team or medical team. The behavioral health care manager is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s medical provider and, when appropriate, other mental health providers.

What is required...

  • One of the following licenses;
    • LICSW
    • LMHC
    • MSW
    • LMFT
    • RN

What is preferred...

  • Previous experience working in a Behavioral Health setting

Additional Information

Wage:  Wage range will depend on license level and years of applicable experience.

FTE:  1.0 = 40 hours/week
