Job Description

POSITION SUMMARY:
The Director of Clinical Navigation Services is a professional nurse who possesses clinical knowledge, teaching skills, and operational oversight. The Director of Clinical Navigation Services will manage clinical and personnel activities of a department or departments within the Navigation Center, which encompasses multiple service lines, navigation processes, and stakeholders, internal and external.  This position may also participate along with other senior personnel to serve as a role model, change agent, and facilitator to provide mentoring and leadership development to establish a collaborative and quality based patient (member) experience in an effort to enhance and promote an effective, Accountable Care Organization environment. The role also encompasses consultation, continuing education, nursing research, professional practice, continuous quality improvement and clinical teaching. This position has the authority, accountability and responsibility, under direction of the Chief Revenue Officer (CRO), to plan, organize, promote, and evaluate multidisciplinary programs and services for the novice and expert clinical employee according to identified learning needs and regulatory/accreditation standards so that quality care, treatment and services are provided for staff/patients/members in an innovative manner that improves the quality and delivery within the care continuum. 


RESPONSIBILITIES:
• Provide direct clinical oversight of Clinical Interventionists and other personnel as defined
• Manage personnel to include interviewing, hiring, counseling, disciplining, and evaluation of employees; working in collaboration with the Human Resources Department and management when necessary
• Provide appropriate training, education, and development to staff as necessary
• In partnership with the Chief Revenue Officer and Director of Navigation Center, provide operational and management oversight to the Navigation Center as needed and/or defined
• As needed and/or directed, participate in clinical reviews, chart audits, and various stakeholder audits
• Maintain full knowledge of NextGen EPM and EMR, including but not limited to designated Superuser status
• Maintain full knowledge of HIE, Patient Portal, Provider Portal and other technologies related to Navigation, to provide analysis, direction/input on future releases and other technologically advanced strategies that enhance the navigation services
• In conjunction with Navigation Center and Medical Leadership, develop processes and procedures to proactively manage chronic disease and other programs as defined, provide education, and help prevent avoidable healthcare expenditures such as Emergency Department visits, hospital admissions, hospital re-admissions, etc.
• Develop, refine, and implement automated processes where applicable
• Prepare status reports and/or implementation plans of programs and services
• Serve as the liaison between providers and other personnel as needed
• Communicate and collaborate with designated personnel regarding service problems, response to community needs, enhancing public relations, coordinating activities and plans, and promoting health programs
• In collaboration with designated personnel will participate in the development/revision of policies and procedures as necessary
• Participate and assist in facilitating the continued development, operation and adoption of a quality navigation system that meets and exceeds standards
• Maintain working knowledge of requirements of various accredidating or regulatory agencies
• Identify the nature of problems, implement solutions, observe and evaluate the outcomes of a problem solution to identify lesson learned or redirect efforts
• Create systems to monitor data and trends and make recommendations for improvement as appropriate; monitor change results and communicate to the key stakeholders
• Perform other related duties incidental to the work described herein as may be assigned or delegated

 

Qualifications

KNOWLEDGE, SKILLS AND ABILITIES:
• Ability to deal effectively with patients, medical staff, administration and the public
• Excellent oral and written communication skills
• Excellent skills in planning, organizing, and delegating
• Knowledge of business and management principles involved in strategic planning, resource allocation, human resources modeling, leadership technique, production methods, and coordination of people and resources
• Skills in organizational development, personnel management, budget and resource development, and strategic planning; demonstrated success developing and monitoring systems to manage both operational and programmatic work that involve high levels of collaboration
• Excellent people skills, with an ability to partner with a dynamic leadership team
• Personal qualities of integrity, credibility, and commitment to the mission of a nonprofit healthcare organization
• Flexibility and an ability to multi-task; able to work within an ambiguous, fast-moving environment, while also driving toward clarity and solutions; demonstrated resourcefulness in setting priorities and guiding investment in people and systems
• Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems
• Monitoring/assessing performance of staff and operations to make improvements or take corrective action

EXPERIENCE, EDUCATION, AND LICENSE:
• Bachelor’s Degree in Nursing or related field preferred
• Current Texas Registered Professional Nursing License required
• 5 years Relevant Clinical Experience required
• BCLS certified required
• 5 years management experience in a Clinic/Outpatient environment required

Our Patients

 

In 2011, we provided nearly 300,000 patient visits. Approximately 50 percent of those patients are children and young people under the age of 20. Lone Star Circle of Care also meets the healthcare needs of pregnant women — delivering more than 1,200 babies last year — as well as seniors, working families, patients with chronic conditions such as diabetes and asthma, and many other patients who have difficulty finding access to health care.

 

Some of LSCC’s patients have public insurance (Medicaid, CHIP, etc.), but are unable to find a primary care doctor who will see them. Nearly 90 percent are underinsured or have no health insurance at all.  Many of the uninsured are hardworking families that simply can’t afford coverage; in Texas 79 percent of uninsured adults work or have at least one working family member. Through Lone Star Circle of Care these hardworking Texans can obtain healthcare services that improve their lives and reduce the overall cost burden of the entire healthcare system.
