Job Description

Intake Coordinator is a qualified, versatile individual who will perform a vast array of administrative and customer service functions, which support a Home Health Agency to assist/support patients from referral to discharge.

ESSENTIAL FUNCTIONS INCLUDE BUT ARE NOT LIMITED TO:

1) Receive and process patient referrals and gather all required information and documents needed.

2) Make decisions choosing the best matched therapist for the referral based on patient needs, therapists’ skills, therapists’ productivity, and location.

3) Communicate with caregiver, referral source, therapist, clinical managers daily via phone and email to solve patient care issues.

4) Prepares and faxes physician notifications.

5) Processes orders from medical software to the physician and receives same orders when signed.

6) Manages discharge documentation and notifications.

7) Assist with eligibility checks on the 1st and 15th of every month at 7:30am.

8) Serve as a translator, as needed.

9) Maintain a positive and professional attitude at all times.

10) Be flexible and able to perform a wide variety of administrative support functions as directed.

11) Performs all duties in compliance with agency policies and procedures.

Qualifications

1) High School graduate

2) Multi-linguistic skills: ability to speak, read, and write English and Spanish-required

3) Ability to multi-task with above average organizational skills-required.

4) Excellent communication skills-required

5) Excellent administrative, organizational, and phone skills-required.

6) Strong Working knowledge of MS Word and MS Excel

7) Experience in medical front office, Medicaid office, in-take department, or doctor’s office-preferred.

8) Must be able to provide reliable transportation for local traveling within the Metroplex.
