Park Avenue Center provides evidence based gender specific drug and alcohol treatment in south Minneapolis, Minnesota. First established in 1979, Park Avenue Center is a community-based program that proudly serves Minnesota’s less fortunate individuals in beautiful settings. Clients we serve typically struggle with multiple barriers including mental illness, homelessness, financial problems, legal issues, and an overall lack of emotional support. Utilizing a multidisciplinary approach, our staff includes Licensed Alcohol and Drug Counselors, Licensed Psychologists, Registered Nurses, and a consulting Family Psychiatric Mental Health Nurse Practitioner who is qualified to prescribe medications. Our robust Mental Health Team allows us to help clients address their various needs and our EAGALA certified staff enables us to provide unique experiential therapy in a calming countryside location. Job Description

JOB SUMMARY

The Case Manager works with clients to obtain Medical Assistance or other insurance; facilitate medical appointments, mental health appointments, and other needed resources. The Case Manager works in conjunction with the Primary Counselors to help obtain stable supportive housing and additional resources for clients upon transitioning out of treatment. In addition, the Case Manager conducts Aftercare Planning Groups with clients and coordinates with outside resources.

 

II.        DUTIES AND RESPONSIBILITIES

  • Develop and maintain up to date resource information for client use.
  • Participates in client staffing to maintain consistent communication with counseling staff.
  • Meet individually with clients determining needs and facilitating resources; helping clients in their transition from residential programming.
  • Facilitate “Resource Group” for clients providing guidance and resources upon their transition from the residence.
  • Meet with families, probation, and social workers to develop transition and contingency plans.
  • Meet with outside resources to determine appropriateness for clients.
  • Maintain appropriate charting to communicate client’s transitional plans as well as outside contacts.
  • Maintain working knowledge of community resources available to clients. Including: health insurance options, housing, medication resources, support groups
  • Coordinate correspondence with Probation, CPS, and other concerned persons.

Qualifications

QUALIFICATIONS

  • Must be LSW or MSW, currently licensed in the State of Minnesota.
  • Computer literacy is required; proficiency is desirable but not required.
  • Must have good typing (word processing) skills.
  • Must have a working knowledge of issues regarding confidentiality.
  • At the time of hiring, must be free of chemical use problems for at least the two years immediately preceding and sign a statement attesting to that fact.
  • Must be able to maintain positive public relations with referring agencies and business community at large.
  • Must possess good verbal skills and be capable of presenting positive professional image.
  • Must pass background check through the Department of Human Services.
