Park Avenue Center provides evidence based gender specific drug and alcohol treatment in south Minneapolis, Minnesota. First established in 1979, Park Avenue Center is a community-based program that proudly serves Minnesota’s less fortunate individuals in beautiful settings. Using evidence-based best practices, we have maintained a reputation for the highest quality of services at extremely low rates. Clients we serve typically struggle with multiple barriers including mental illness, homelessness, financial problems, legal issues, and an overall lack of emotional support. Utilizing a multidisciplinary approach, our staff includes Licensed Alcohol and Drug Counselors, Licensed Psychologists, Registered Nurses, and a consulting Family Psychiatric Mental Health Nurse Practitioner who is qualified to prescribe medications. Job Description

JOB SUMMARY

Under the supervision of the Clinical Supervisor, the primary role of the Client Care Coordinator is to monitor the activities of Park Avenue Center’s Complex Co-Occurring clients during the course of their shift. This individual also serves as a liaison between clients and Program Staff.

DUTIES AND RESPONSIBILITIES

1. Serves as a resource for clients and is responsible for maintaining order during the course of their shift.

2. Facilitate primary groups and individual sessions as outlined.

3. Participates in client staffing and maintains consistent communication with Program Staff regarding client progress and/or issues or incidents that may occur with clients.

4. Monitor clients providing urine for analysis.

5. Transportation of clients to and from treatment, court, activities, and/or other approved appointments.

6. Performs walk-throughs of properties on a rotating basis during the course of their shift.

7. Performs house inspections for cleanliness and general property upkeep.

8. Must have knowledge of and comply with client rights, outlined in part 148f.165 of the Minnesota Administrative Rules and of staff responsibilities.

9. Maintain professional relationship with clients while adhering to Park Avenue Center client service guidelines and professional code of ethics.

10. Other duties as assigned.  

Qualifications

1. Bachelor's degree in psychology, social work or human services related field.

2. Master’s degree in psychology, social work or human services related field preferred but not required.

2.Must comply with requirements for licensure or board certification as a mental health professional including supervised practice in the delivery of mental health services.

3.Must possess a valid, unrestricted MN Driver’s License.

4.Must possess good verbal skills and present a positive professional image.

5.At the time of hiring, must be free of chemical use problems for at least the two years immediately preceding and sign a statement attesting to that fact.

6.Must pass a background check through the Department of Human Services (DHS).

Additional Information

This is a part-time position with 24 - 29 hours a week with shifts varying in early morning, afternoon, evening and weekends.
