Job Description

The LPN functions in the Medication Observer role at Park Avenue Center monitoring medication compliance among clients. the LPN processes new client information, including accepting medications prescribed for these individuals and entering them into the electronic medical record, documents and maintains accurate records of medications (including controlled) taken by clients, participates in medication destruction and other nursing actions as directed by the Supervising RN.

DUTIES AND RESPONSIBILITIES

1. Record and provide detailed description of medications taken by clients in CODP

2. Participate in client admission assessing medication brought to the sites for correct labeling, clarity of directions, source for refills and client understanding of reason for taking the medication and compliance

3. Maintain records for medication destruction, controlled medications and routine audits as requested by RN

4. Coordinate medication refills and new medication orders with clients and pharmacies

5. Observe clients for signs and symptoms of exacerbation of existing chronic conditions, adverse reactions to medications, and other physical needs as clients or staff request; notify RN and contact clinical staff for client needing medical intervention or coordinate emergency services as needed

6. Maintain professional relationship with clients while following a strict code of ethics

7. Perform other duties as assigned


Qualifications

1. Must have a High School Diploma and have graduated from a Practical Nursing program recognized by the State of Minnesota

2. Must possess Minnesota licensure to practice as a Practical Nurse

3. Must meet the staff training requirements in the company's policies

4. Must have knowledge of and comply with client rights, outlined in pare 4747.1500 and of staff responsibilities

5. Must have strong communication skills and present a positive, professional image

6. At the time of hire, must certify free of chemical use problems for at least two years immediately preceding and sign a statement attesting to that fact

7. Must pass a background check
