Park Avenue Center provides evidence based gender specific drug and alcohol treatment in south Minneapolis, Minnesota. First established in 1979, Park Avenue Center is a community-based program that proudly serves Minnesota’s less fortunate individuals in beautiful settings. Clients we serve typically struggle with multiple barriers including mental illness, homelessness, financial problems, legal issues, and an overall lack of emotional support. Utilizing a multidisciplinary approach, our staff includes Licensed Alcohol and Drug Counselors, Licensed Psychologists, Registered Nurses, and a consulting Family Psychiatric Mental Health Nurse Practitioner who is qualified to prescribe medications. Job Description

Under the supervision of the Chemical Health Assessor Supervisor, the Chemical Health Assessor completes Rule 25 Assessments, Chemical Health Assessments, and other duties as assigned.

DUTIES AND RESPONSIBILITIES

1. Conduct all aspects of Rule 25 Assessments

2. Sign and adhere to all requirements outlined in Hennepin County’s Memorandum of Agreement.

3. Maintain all needed correspondence with the Hennepin County review team, treatment facilities, child protection workers, probation officers, collateral contacts, and managed care organizations as applicable.

4. Fulfill periodic tasks such as answering the phone, addressing visitors at the front door, or help coordinate clients getting to their proper group room or office.

5. Continue to upgrade skills via educational media.

6. Provide services on a limited basis to client’s families and/or significant others as needed.

7. Maintain positive, upbeat and professional attitude with the community remembering that you are the “face” of Park Avenue Center.

8. Maintain absolutely professional relationship with clients while following a strict code of ethics.

9. Perform other duties as assigned.

Qualifications

1. Must have a Licensed Alcohol and Drug Counselor license under Minnesota Statutes, Chapter 148C, have the ability to receive a temporary permit, or meet Rule 31 licensing requirements for qualified staff by other means.

2. Counselors exempt under Minnesota Statutes, chapter 148C, must be competent, as evidenced by one of the following: (a) completion of at least a baccalaureate degree with a major or concentration in social work, nursing, sociology, human services, or psychology, or licensure as a registered nurse; successful completion of a minimum of 120 hours of classroom instruction in which each of the core functions listed in Minnesota Statutes, chapter 148C, is covered; and successful completion of 440 hours of supervised experience as an alcohol and drug counselor, either as a student or as a staff member; (b) completion of 270 hours of alcohol and drug counselor training in which each of the core functions listed in Minnesota Statutes, chapter 148C, is covered, and successful completion of 880 hours of supervised experience as an alcohol and drug counselor, either as a student, or as a staff member; (c) current certification as an alcohol and drug counselor or alcohol and drug counselor reciprocal, through the evaluation process established by the International Certification and Reciprocity Consortium Alcohol and Other Drug Abuse, Inc., and published in the Case Presentation Method Trainer's Manual, copyright 1993. The manual is incorporated by reference. It is available at the State Law Library, Judicial Center, 25 Reverend Dr. Martin Luther King Jr. Blvd., St. Paul, Minnesota 55155; (d) completion of a bachelor's degree including 480 hours of alcohol and drug counseling education from an accredited school or educational program and 880 hours of alcohol and drug counseling practicum; or (e) employment in a program formerly licensed under parts 9530.5000 to 9530.6400 and successful completion of 6,000 hours of supervised work experience in a licensed program as an alcohol and drug counselor prior to January 1, 2005.

3. Must document competence in screening for and working with clients with mental health problems, through education, training, and experience.

4. Maintain comply with the rules adopted under MN Statutes, chapter 148C.

5. Must possess good verbal skills and be capable of presenting positive professional image.

6. At the time of hiring, must be free of chemical use problems for at least the two years immediately preceding and sign a statement attesting to that fact.

7. Must pass a DHS background check.

8. Must be trained by Hennepin County on how to complete Rule 25 Assessments.

9. Must sign the Hennepin County Memorandum of Agreement for completing Rule 25 Assessments.

WORK ENVIORNMENT
1. Traveling may be required daily
2. Carrying a laptop and other supplies with a weight of up to 15 lbs
3. The office environment includes a typical office environment with limited exposure to dust
4. Other environments may include small spaces, windowless rooms, jails, hospitals, and clinics including locked units
5. Performing work on a computer is required up to 8 hours per day

This is a full-time, benefits eligible position working Monday-Friday typical business hours. Some travel may be required.
