Since 1976, St. Joseph Center has been meeting the needs of low-income and homeless individuals and families in Venice, Santa Monica, Mar Vista, and surrounding communities.  The Center is a 501(c)(3) nonprofit community organization that assists people without regard for religious affiliation or lack thereof through comprehensive case management and integrated social service programs.  The Center enjoys broad-based community support as well as a sponsored relationship with its founders, the Sisters of St. Joseph of Carondelet.  St. Joseph Center serves approximately 6,000 individuals annually.

Brief Description of Principal Activities

Multifaceted intervention, prevention, and education services are carried out at four sites on the Westside of Los Angeles.  St. Joseph Center’s integrated programs provide clients with concentrated and coordinated access to services according to the nature of their needs.

Case Manager
Volunteers of America, Los Angeles
Supportive Services for Veteran Families (SSVF)
The Case Manager reports to the Lead Case Manager or designee.  The Case Manager will arrange, coordinate, monitor and deliver services related to meeting the needs of SSVF program veteran households, while helping them obtain housing stability.

In order to accomplish this goal, the Case Manager will:
•    Manage a caseload of clients; maintain case files
•    Conduct comprehensive assessments to collect functional, environmental, psychosocial, financial, employment, housing, educational and health information as appropriate to develop a case plan.
•    Develop support systems to meet client needs by identifying and coordinating a variety of available services necessary to maintain independent living, and, when possible, self-sufficiency and family stabilization.
•    Collect necessary documentation for SSVF verification purposes
•    Provide supportive counseling and advocacy for clients
•    Refer veteran household to available VA/Community providers for therapeutic, social, educational and medical resources when appropriate.
•    Conduct crisis intervention as necessary.
•    Monitor and verify services provided to each veteran household on a regular basis, determining quality and effectiveness of services provided.
•    Conduct new veteran household intakes and ensures all supporting documents are verified/filed.
•    Interview veteran household for emergency assistance and determines eligibility.
•    Document and maintain case management files for all veteran households.
•    Monitor and document progress of veteran households.
•    Administer direct assistance of funds by way of third party check payment.
•    Provide information and referrals for callers and walk-in veteran households.
•    Develop, perform, and assist with workshops pertaining to veteran household needs.
•    Participate and assist with outreach events.
•    Other duties as assigned.

MINIMUM QUALIFICATIONS:

Bachelor’s Degree required.   Minimum of one year experience, with the homeless population, in case management services with demonstrated skills in working with clients form various backgrounds; embracing our mission providing basic human needs with compassion and in a dignified environment.

Must be motivated with a sense of teamwork and possess good communication and people skills; Demonstrate proficiency in MS Word, Excel and internet usage. Possess database skills; HMIS a plus. This position is subject to a background check.
