Since 1976, St. Joseph Center has been meeting the needs of low-income and homeless individuals and families in Venice, Santa Monica, Mar Vista, and surrounding communities.  The Center is a 501(c)(3) nonprofit community organization that assists people without regard for religious affiliation or lack thereof through comprehensive case management and integrated social service programs.  The Center enjoys broad-based community support as well as a sponsored relationship with its founders, the Sisters of St. Joseph of Carondelet.  St. Joseph Center serves approximately 6,000 individuals annually.

Brief Description of Principal Activities

Multifaceted intervention, prevention, and education services are carried out at four sites on the Westside of Los Angeles.  St. Joseph Center’s integrated programs provide clients with concentrated and coordinated access to services according to the nature of their needs.

Job Description

SPA 5 Coordinated Entry System (CES) Matcher
Full Time: Non-Exempt

The Coordinated Entry System (CES) is being developed to more efficiently connect homeless single adults searching for permanent housing to the most appropriate housing resource. Coordinated Entry System is a way by which persons experiencing homelessness and organizations providing housing find each other in a systematic and efficient manner. Under supervision, the CES Matcher is responsible for the following activities: Facilitate the prioritization process. Work with CES team to create efficient housing process Adapt process when system barriers are identified Ensure collective impact model is implemented among agencies Manage information in the SPA 5 CES database Using CES database to match individuals document ready to appropriate housing Notify identified Agency Navigator that match has been made Track acceptance of match made and document in CES database Confirm housing outcome by documenting in CES database Provide on-going support to SPA 5 Agencies regarding the CES database – support at times will require travel to agencies in the community. • Provide community outreach in SPA 5 to engage new partner agencies Attend all meetings required by Home For Good/CES Administrators Attend all SPA 5 CES meetings – both executive and provider meetings In consultation with the CES team, create standard CES database protocol to ensure integration among SPA 5 agencies. Data Analysis o Run Weekly Data Quality reports to ensure agencies are entering data correctly Work with CES agency staff to fix any data quality issues Create Reports as directed • Training  Remain informed on CES database updates and train partner agencies on any changes Participate in all SPA wide CES Matcher trainings Train and engage new agencies as they become SPA 5 CES partners Enter VI-SPDATs into database when directed Provide on-going training to SPA 5 staff on the VI-SPDAT assessment tool Other duties as assigned by the CES Team.

QUALIFICATIONS: Knowledge and use of computers and software including but not limited to Excel, Word, Access, SQL, and HTML required. Experience with relational database design and business intelligence reporting software required. Ability to multi-task, and attention to detail and organizational skills required. Previous HMIS experience, and understanding of SPA 5 providers and larger Continuum of Care preferred. Good oral and written skills necessary. B.A. Degree preferred.   This position is subject to a background and DMV check.
