Job Description

The Physical Therapist is responsible for supervising and participating in the application of a wide variety of physical therapy treatments to enhance rehabilitation of physical disabilities, injuries, and disease processes. The Physical Therapist evaluates, plans, and administers comprehensive physical therapy modalities/regimes to patients in their place of residence for the purpose of developing and restoring function through rehabilitative physical therapy as prescribed by a physician. The Physical Therapist complies with applicable laws and regulations and adheres to Medicare and Medicaid regulations.

PRIMARY JOB DUTIES & RESPONSIBILITIES

· Performs comprehensive assessment utilizing the OASIS data set.

· Performs initial and ongoing patient assessments in response to patients’ needs and in accordance with physician orders.

· Initiates a written plan of care, re-evaluates and updates the plan as necessary.

· Provides physical therapy services as prescribed by a physician, which can be safely provided in the home and assisting the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional abilities test within the therapy scope of practice.

· Assists the physician in evaluating disability, injury or disease process of individual patients, and in determining the patient’s level of function.

· Interprets plans and establishes and implements the proper treatment in accordance with the physician’s orders.

· Contacts the physician directly for patient reports and care as needed.

· Submits appropriate documentation of the patient’s progress and course of care to the agency and the physician.

· Documents skilled care and/or services provided in compliance with federal and state laws and regulation, and agency policies and procedures.

· Observes and records activities and findings in the clinical record and reports to the physician and Director of Nursing/clinical designee the patient's reaction to treatment and any changes in patient's condition, or when there are deviations from the plan of care.

· Instructs the patient and caregiver in care and use of physical therapy devices.

· Instructs other health team personnel including, when appropriate, Home Health Aides and caregivers in certain phases of physical therapy with which they may work with the patient.

· Evaluates the home environment and makes appropriate recommendations.

· Supervises Physical Therapy Assistants and Home Health Aides as required. The physical therapists shall be accessible at all times by two way communication, which enable the Physical Therapist to be readily available for consultation during the delivery of care.

· Instructs the patient and caregiver on the patient's total physical therapy program.

· Attends agency meetings and in-service programs as required.

· Acts as Case Manager for therapy-only cases.

· Participates in case conferences to discuss multidisciplinary team responsibilities, patient progress, and plans for continued care.

· Promotes and provides patient/family/caregiver education using various verbal and written communication techniques that take into account the patient’s/family’s cultural, ethnic, and/or personal needs or preferences.

· Plans patient discharge appropriately; provides information about community resources to address patient ongoing needs.

· Participates in the agency’s performance/quality improvement program.

· Obtains appropriate number of continuing education credits to maintain re-licensure status.

· Completes projects/assignments within specific timetables.

· Attains goals as set forth in the annual performance evaluation.

· Submits documentation within required timeframes.

· Maintains strict confidentiality on patient, agency, and employee matters.

· Performs other duties as assigned.

Qualifications

Graduate of a physical therapy curriculum approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals of the American Medical Association, or both.

If licensed or registered prior to 1966, you must meet the alternative qualifications cited in the Medicare Conditions of Participation, Section 405.12.

If trained outside the United States:

Graduated after 1928 from a physical therapy curriculum approved in the country in which the curriculum was located and in which there is a member organization of the World Confederation for Physical Therapy. Must meet the requirements for membership in a member organization of the World Confederation for Physical Therapy.

Licensure:

Currently licensed in the state of practice

Experience:

Minimum of 1 year experience as a Physical Therapist
