Job Description

We are excited to announce a new position in our Guest House! The new Clinical Nurse Manager’s top priorities will be to lead, supervise, support and engage our nurses. Our nurses are the cornerstone of our unique Zen Hospice Project care model. They are trained in end of life care best practices - including advanced symptom management rooted in mindful and compassionate care-giving modalities - that strive to create a compassionate, genuine, comprehensive and respectful approach to those facing the end of their lives. They serve our residents with uncompromising attention to detail, and deserve a leader who can do the same for them. Example responsibilities of the Clinical Nurse Manager include:

Supporting clinical staff of 12+ including RNs, LVNs and CNAs

Providing clinical oversight of the Guest House and direct patient care (as needed)

Formulating, organizing, and directing resident plans of care

Working closely with the Referral and Admissions Coordinator to cultivate community partnerships, establish a robust referral list and coordinate care with outside agencies, hospitals, and hospice programs

Assisting with the intake process by travelling to local hospitals, community clinics, and private homes to assess patients for admission to ZHP

Recruiting and training new nurses including onboarding to ZHP

Evaluating continuing education needs and develop and implement in-service trainings

Coordinating adequate daily staffing

Ensuring organizational compliance with legal, regulatory and accreditation requirements, including serving as a liaison with the Department of Social Services (DSS)

Qualifications

Graduate of an accredited RN training program with current licensure as a Registered Nurse or Advanced Practice Nurse

3+ years of nursing experience specializing in palliative/end-of-life/hospice care

2+ years of clinical management experience

CHPN or PHN desired

In-depth knowledge of and commitment to ZHP’s mission and philosophy
